Enrolment
We are now taking enrolments for 2027!
Parents can complete an Application for Enrolment form at the school office, or downloaded below.
Documents to be provided with your application:
1. Your child’s birth certificate,
2. Australian Immunisation Statement printed in the last 2 months, and
3. Proof of address, such as an utility bill, driver's licence, rental agreement or property title.
All completed forms can either be emailed at mounthelena.ps@education.wa.edu.au to the school or posted to Lot 289a Keane Street East, Mount Helena 6082.
Application for Enrolment Forms
